Surgiyo LLC (together with any affiliates, “Surgiyo”) provides a multi-platform service for surgeons and other
healthcare professionals to upload, host and edit surgery videos in the cloud. This Privacy Policy describes how Surgiyo collects, uses, or discloses personal information we receive through a browser, mobile app and any related online or offline services that we provide (collectively, the “Services”). This policy also describes your choices about the collection and use of your information. To the extent you have entered into a contractual agreement executed by Surgiyo, the terms of that agreement will control in the event of any conflict. Please read the Privacy Policy carefully before you start to use our Services. By using the Services, you agree to be bound and abide by our posted Terms of Use (“Terms”), which incorporate this Privacy Policy by reference. If you do not agree to our Terms and this Privacy Policy, or if you violate them in any way, your right to access or use the Services is terminated. How We Collect Information
We may collect information about you by various means, including:

- Profile and subscription information, such as your name, mailing address, e-mail address, phone number, usage and subscription preferences, professional title and role, user credentials for any access-restricted Services, and payment information if you pay any fees or subscriptions.
- Organizational information, such as information provided by an organization that your account or profile is associated with;
- Survey information in response to questions that we may ask you through the Services, including for research purposes or to improve our Services;
- User Content that you may provide to us via the Services (e.g., information, photos or videos you may post to the Services, social media content, testimonials, and other materials) along with any associated information that you provide about yourself or the submitted content;

Online User Activity, Cookies and Information Collected by Other Automated Means
Cookies are a commonly-used web technology that allow websites or mobile apps to store and retrieve certain information on a user’s system, and track users’ online activities. We and our service providers may collect information about your use of our Services by such automated means, including but not limited to cookies, pixels, SDKs and other similar technologies. Cookies and similar technologies can help us automatically identify you when you return to our website or app. Cookies help us review traffic patterns, improve our Services, and determine what Services are popular.
